Chris CoutinhoandClaude Opus 4.6 29fd0486c9 refactor: change OAuth scope separator from colon to dot for IDP compatibility
Many identity providers (AWS Cognito, Okta, Azure AD) reject or mishandle
colons in OAuth scope names. This migrates all custom scopes from
`resource:action` to `resource.action` format (e.g., `notes:read` →
`notes.read`), which is universally accepted and aligns with industry
conventions (Microsoft, Google).

Includes Alembic migration 004 for stored scope strings and ADR-024
documenting the rationale and RFC references.

Add support for one-time use refresh tokens with automatic rotation
to align with Nextcloud OIDC security model.

Changes:
- TokenBrokerService improvements:
  - Add user_id parameter to refresh methods
  - Detect and store rotated refresh tokens
  - Add offline_access scope to token requests
  - Handle refresh token rotation on every use
- Add management API endpoints:
  - /api/v1/webhooks (GET/POST) - List/create webhooks
  - /api/v1/webhooks/{id} (DELETE) - Delete webhook
  - /api/v1/search (POST) - Unified search
  - /api/v1/chunk-context (GET) - Get chunk context
  - /api/v1/apps (GET) - List installed apps
- Update tests for refresh token rotation
- Add --headed flag to pytest for Playwright debugging

- Aligns with Nextcloud OIDC one-time refresh token model
- Prevents refresh token invalidation after first use
- Enables long-lived background operations

- Token Broker Service manages Nextcloud access tokens with audience validation
- Implements short-lived token caching (5-minute TTL) with early refresh
- Enhanced token storage schema with ADR-004 fields (flow_type, audience, provisioning)
- MCP provisioning tools for explicit Flow 2 resource authorization
- Comprehensive unit tests for Token Broker Service (14 tests, all passing)
- Environment configuration for Progressive Consent mode

This implements the foundation for the dual OAuth flow architecture where:
- Flow 1: MCP clients authenticate to MCP server (aud: "mcp-server")
- Flow 2: MCP server gets delegated Nextcloud access (aud: "nextcloud")

Users must explicitly call provision_nextcloud_access tool to grant resource access,
implementing the "stateless by default" principle from ADR-004.
